II  INDEPENDENT  LIVING  TRAINING
      and
      SUPPORTED LIVING SERVICES


FHAR provides independent living training and supported living services to graduates of FHAR's Level III Residential Program, individuals residing in their own homes or with their families, and to those living in FHAR facilities. Independent and Supported Living Services have three components:


A.     Independent  Group  Residences  (IGRs)

           East Poplar Avenue, San Mateo
           El Camino Real, Burlingame
           El Camino Real, South San Francisco (2)

           Palm Avenue, Redwood City (3)
           Oakley Avenue, Menlo Park

FHAR operates eight IGR facilities, which are semi-independent shared housing arrangements that provide independent supported living services. Individualized programming is provided for each consumer/resident


B.     Berkshire  Apartments

This apartment complex is designed to provide housing and training for individuals living independently. Individualized follow-up support services are provided to Berkshire clients to maintain their community living skills.


C.     Community Outreach Program

FHAR provides independent living training and supported living services to individuals who reside in their own home and/or apartment. This Community Outreach Program is provided to consumers throughout San Mateo County.

FHAR also provides support services to individuals residing with their families. The programmatic goal is to assist clients to live more independently in their family's home or to prepare them to transition to a more independent living arrangement.
